Membership Cancellation Request


The Performance Lab requires a 30 day cancellation notice. If the member wishes to cancel this agreement on the terms set out, they may do so by submitting a cancellation notice form that can be found on The Performance Lab website ( or in person. Your membership will be cancelled 30 days after your next scheduled payment.

Note that if you have a scheduled renewal payment with in time of cancellation notice the payment will be processed as scheduled. All payments are non-refundable. Member will enjoy membership benefits until the end of the then-current membership term for which you have paid. Agreements are auto renewed unless the membership cancellation form is submitted to us at least 30 days prior to your scheduled payment.

Any cancellation form submitted on a non-business day will go into effect until the following business day. The only acceptable methods to submit a cancellation are via our website form or in person.
